The site continues to grow in depth and we appreciate any comments that you have regarding useful information that you would like to see added.

Important Reminder: Although most of our employees work at Beth Israel Deaconess Medical Center or an off-site location, it is important to remember that you are an employee of HMFP/APHMFP/CAPHMFP when it comes to personal questions regarding benefits, payroll and employment verification.

Applicant and Employment Information

OPEN ENROLLMENT 2012
The 2012 Benefits Open Enrollment Site (www.eelect.com) will be open beginning October 31st through November 11th, 2011. We are asking that all active employees take time to review the information and make their selections. This year there are number of changes that are important to review. Information on the new BIDMC Choice Plan (POS) and HPHC PPO can be found using the links above. All newly hired or returning members to HMFP/APHMFP/CAPHMFP will take part in a personal meeting to review and complete all appropriate forms for employment.

Benefits/Payroll
Information on each Benefits program can be found by clicking on the links above. Annual re-enrollment typically happens each November with an effective date of January 1st. Please refer to your open enrollment materials for additional information.

Email Distribution: In an effort to get you information that is both informative and required by law, we have created a distribution list. We recognize the volume of email that one gets these days, and as such, try to limit the announcements to once a month. However, it is important to recognize that from time to time emergency updates are needed. We appreciate your attention to these notices. From time to time notices for BIDMC employees regarding benefits and payroll are disseminated via Corporate Communications, if your Organization is not specifically referenced then you are not impacted by these updates.

Required Forms: Most changes to benefits and payroll can be done by email and/or faxing of a required form. Click here to link to the most commonly used forms for payroll and benefits. The fastest form of communication within the department has been found to be email.

Many of the forms found on the website can be completed and returned via the confidential fax number below.

Workers Compensation
If you should experience a work related injury you should seek medical attention immediately. HMFP employees during business hours report to Employee Occupational Health (EOH) LMOB 6th floor, if after business hours report to Emergency Department.

If you are APHMFP or CAPHMFP employee you should report directly to an Emergency Department. A First Report of Injury form MUST be completed for all workers compensation injuries. Termination Process
If you are terminating your employment you will receive a termination packet in the mail from the Human Resources Office. A termination letter is included in the packet and it will provide you with details on what happens to your benefits upon termination.
